TRIA forms are company specific; for information on selection of
which form to use, please see below. For PERSONAL LINES, AUTO,
-or- GARAGE coverage, no TRIA form is required.

TRIA_SIC_08-06 - Scottsdale Insurance Company
TRIA_WHIC_01-08 - Western Heritage Insurance Company
Selection of Forms to use:There are a number of things to consider when
selecting a TRIA form. The Company is listed on the quote (new
or Renewal) you received from us, and is located a few lines below
the shadowed box addressing TRIA coverage, and below any forms
applicable to the policy. Policy number is the next place to
look. This table has a listing of common policy number prefixes
we write and
list the company's form to use (not all policy prefixes listed).
